Submit your Consent to Join Form

To join the settlement online, first verify who you are using the ILYM ID printed on the notice mailed to you and your last name. You will then be taken to your Consent to Join Form to review and sign.

Your ILYM ID is printed on the Notice of Settlement and Consent to Join Form mailed to you. If you cannot find it, contact the Settlement Administrator.
